Along with a translation if possible.\n<ul class=\"wp-block-list\">\n<li>One of the following: A long-form birth certificate, a short-form birth certificate showing your parents\u2019 information (or any document issued by a consulate), a family record book (\u00ab livret de famille \u00bb), a certificate of marriage.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Residence permit or document validating long stay visa (VLS)<\/li>\n\n\n\n<li>A Bank identity statement (in French: RIB) or an IBAN<\/li>\n\n\n\n<li>Parental authorisation statement for children under 16 years of age<\/li>\n\n\n\n<li>If necessary, additional documents according to the country of origin<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\">Then create your personal account:<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Download your temporary Social Security registration certificate, and <strong>you will immediately have access to the reimbursement of your health expenses <\/strong>by the French health insurance.<\/li>\n\n\n\n<li>Upload any missing or incomplete documents to your personal account.<\/li>\n\n\n\n<li>Once you receive a message with an NIR number, you can download your definitive social security affiliation certificate.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Open your personal ameli account:\u00a0<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Open your <strong>personal ameli account <\/strong>by calling 3646 (French) or 0974 753 646 (English). Your bank details (RIB), social security number and a code received by text message will be necessary to create your <strong>personal ameli account<\/strong>.<br><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Once your ameli account is opened, you can:\u00a0\u00a0<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Request your \u201ccarte Vitale\u201d by providing your ID documents: recent passport photo and ID papers.<\/li>\n\n\n\n<li>Register a primary care physician (\u201cm\u00e9decin traitant\u201d) during a consultation with a doctor in France<\/li>\n<\/ul>\n\n<\/div>\n\t\t<\/div>\n\n<div class=\"wp-block-ub-content-toggle-accordion\" style=\"border-color: #f1f1f1;
